For me, I found the game lacking compared to Fallout 3. There is significantly less overall content in the game, and the game is incredibly restrictive about where you can go, especially during the first parts of the game. The new damage threshold as supposed to the old damage resistance system is abused and basically prevents you from deviating from the main path until you work your way around to Vegas for the first time. While there are SOME optional quests, it's definitely not like Fallout 3 where you could just forget the main quest and dick around for a good while.

I've played this game for a while now and I haven't even made it to Vegas yet. I just keep exploring and I've gotten side quests where I've gone. Some from the NCR but mainly from the places I visit. I'm just guessing but it does seem like the equipment I've been able to find and buy has been limited by me not progressing in the main storyline though. Either that or I've been very unlucky. (I just want a sniper rifle!) So far the world area seems to be as big as FO3 was, but it does seem like the cities, buildings, sewers, etc. are not as large of an area to explore as FO3. But I'm only 10-15 hours in, I'm guessing I have another 40-50 hours to go of wandering.
And a side note, if you have the chance to pick up the robot ED as a companion, not only can you also get a human follower as well, but ED's sensors are extremely helpful as they are able to pick up enemies from great distances.
