Kicked for "not following orders"?
On Wave Breaker. Got kicked a few seconds before the round ended with the message I was "not following orders".
The only orders I am aware of not following were in the begging of the round when I and my squad spawned in the chopper. Tenroue, the commander, was giving us orders to cap D, inside the cave flag, which we obviously could not do as we were in the chopper. We capped three of the little island flags before we died and then mainly groundpounded between the 3 cave flags.
I believe I and my squad capped and held the most flags as my 15000+ score for the round would indicate. Here is the battle report:

In many years on TPG through BF2, BFBC2, BF3, I have only been kicked one other time that I recall, other than the auto reserved slot kicks. I do my best to follow orders and thought I was doing that on Wave Breaker. It's possible I was too focused on the battle and missed the orders or warning so clarification would be appreciated.

Did he use VOIP as well as issue orders with the Comma Rose?

It appears to me that the Commander gave you an order you did not follow, repeatedly. I do understand that sometimes it is hard to know that the Commander is giving you an order, via the Comma Rose. I also have empathy for difficulty following the chat messages. However, Tenrou says he used VOIP to give you orders as well.
Score matters nothing if you fail to follow your Commander's or your SL's orders. You are a strong player and can be a tremendous asset on the server. Given your past, I am sure you will endeavor to always follow orders.
I was on the opposite team and felt the brunt of your Comander's strategy as well as your gun. Both frustrated our efforts and crushed our attack.My goal is to succeed in any mission - and live to succeed again.

Only "VOIP" thing was Tenroue jumping in VOIP toward the end of the round and saying "Why are you all on the middle flags? We are losing the ones on the outskirts? and leaving. I thought that's what he had wanted to us to do given the initial orders to attack D when we were capping the outside flags in the chopper.

Sounds like an honest misunderstanding. It happens.
I don't think this has to go much farther. Though it is good to have cleared the air.
We follow orders. If we fail to do so, a kick is the least of our worries. Our admins will ban for this offense too.

I submitted new orders to your squad at least 10x I came into your VoIP channel roughly 3x to ask why you were still attacking middle flags. I waited about 20-30 seconds for a response from you and got nothing. I also put in team chat that all squads should stay on the outiside flags and leave the middle alone. Aside from going to your house and slapping the mouse out your hands I did everything I could from commander chair to get you out of middle flag. So sorry I don't buy misunderstanding. Your lucky you got kicked only. If squad leading while commander is on board is to much for you to manage while playing let someone else do it.

Voips patchy at best, Not a reliable way to convey messages.
Chain of command is there to help the team push, that being said some understanding that the battlefield often has more to it than the commander sees.
Keep up the teamplay guys, that means we all work towards the same end. If the commander has issued a potentially problematic order, respond giving real-time information.
Tenrou can only do and see so much from his iPad on the potty.
Brass was in battle, Sometimes spamming the order command gets their attention, personally I love commanders hopping into voip and stating their arrival (ie "commander here, acknowledge" ) makes it easier to understand what the fuck is happening on the ground level.
Above all please pay attention to your commander and SL .
