I would like to register a complaint
i have lost sleep
the bags under my eyes have grown
my lower lip stretches from lobe 2 lobe from smiling (takes 2 or 3 hours to go back)
my thin skin can barely handle the smack chat
ive laughed food and spit tobacco on my monitor (by accident)

im thinking about coming back but i dont now if i can take anymore

Fill this form out, in triplicate. Make sure you also have any witness's to the event fill out similar forms. Submit these to Merrit, and he will respond to them within 3-7 weeks.

Then get it off to HR and they will direct it to the appropriate Dept. Head , you should receive a reply in 6 to 8 years. Thank You , Document Administration.
