Thats happened to me, Can't remember what part it was ages ago when I was walking through and she just dropped from the ceiling right in front of me.

Another time in DA going through the car park and just as I was going up the hill a tank drops from the ceiling too.

Biggest Pain In The Ass Witch Story:
Left 4 Dead 1 - Cant Remember Campain - Single Player
Was going through a campaign to try and get the startle no witches achievement, and right before the finale room there was a witch sat pretty much right outside the safe room ... completely unavoidable and unleashed some serious nerd rage, it was either me or the bots that woke her up

Biggest Nerd Rage Witch Story:
Left 4 Dead 2 - Dark Carnival - Online Campaign Expert
Was going through dark carnival on expert and I somehow manage to startle EVERY witch, and im no noob so this was just annoying ...
1st Startle: Ran straight past her seemingly easy enough *incoming horde* stopped me from running, bang witch killed me.
2nd Startle: this was in the love boat ride, she was in the middle i ran past, seemed to be a good distance away, shot a few zombies (nowhere near the witch) somehow i startled her being a good distance away with no flash light on...
3rd Startle: I ran past the witch on love boat ride again after we had to restart that stage *RAAWWWWRRR* ... yeah charger ... pushed me right back to the witch ... bam i startle the witch -.-

There was a few other times this happened aswell ... on the SAME campaign ... I nearly threw the xbox out the window.

I was playing L4D this morning and was doing the church bit, I went to the preist guy to alert the horde, door opens and there is a boomer, preist guy is still talking Louis manages to get in the safe room while the rest of us couldn't. Then after the horde the preist guy was still talking when we all went into the safe room.

Annoying Witch spawn yesterday while me and my friend were trying for the Sobs Story achievement in L4D2. We had carefully dodged all the Witches in the sugar mill got in the lift to take us to the field for the final dash to the gas station. As we was going down we could hear some crying getting louder and louder then suddenly the witch raised through the lift floor and attacked me. Seems she had decided to sit or wander under the lift.
