A really good point is to listen to the comm chatter over your team's radio, and the chatter you hear in-game around you. If your commander is speaking in Russian, and you hear someone talking in some stuffy British accent around the corner, make with the buckshot.

I have to say count on the camo more than the names above the head because to often, you see a teammates name (from someone on the other side of the building or something) above your enemy, you don't shoot and they kill you wondering why you didn't shoot.
Also, watch for teammates who pick up a kit from the other team as sometimes you suddenly forget what team you/they are on...also have to learn to look at armor vs no armor when IDing people.

After a while, you'll be able to simply look around, see someone, and immediately know if they're friendly or hostile. It becomes natural after a while. Just keep at it, and keep the fire off the friendlies.
