Thank you so much. I did what I felt was a fairly exhaustive search on the forum (and a few others) before posting here, but it's a difficult problem to describe and so while I came across a few things that sounded similar, I was never certain if they were the same issue or not.
I'll go and install the game and read the thread now.
If my 360 is faulty, it's still under warranty so replacing it won't be an issue.

I now know the problem. It must be the disc. I tried installing and it keeps saying the disc can't be read at the 14-16% mark. Still no visible flaws.
The disc must have come faulty, or else been damaged by the machine. I bought it brand new, popped it in the tray (without scratching it or touching in an incorrect way) and played for a few minutes. Perfect graphics. I quit the game, started a new one in commentary mode and suddenly there was an issue. It was the first game I used in a brand new 360.
So far my other discs seem okay, but now I'm worried my 360 is going to damage them.

If you can't take the disc back to your supplier and get a new one because it's faulty (ie you don't have the receipt)...
Can you borrow or hire another L4D1 disc?
If so, install that disc to the HD, give / take it back, then use your 'broken' disc to boot the game. That should solve the problem and also means you don't have to buy a new copy.
Also, try clearing your cache before that install:
>Guide
>System Settings
>Memory
Highlight you HD and hit 'Y'.
>Clear System Cache
After installing to the HD and when you boot the game, it'll automatically re-download the updates and DLC, so you'll have a totally fresh install of the game.
