I have the exact same issue as RealMirage. I just started having this problem yesterday, and I had been playing the game with no issues since release. Doesn't even start up, just crashes. I updated my drivers and tried just about everything I could think of.
NVidia GeForce 7600 GS 512MB
Intel Pentium D CPU 2.80GHZ (Dual core)

Thanks, but I tried re-downloading and re-installing L4D this morning...no dice. I didn't have any sound or lag issues to speak of either. Re-installing steam doesn't seem like it would fix anything as it's the executable itself that's crashing after Steam successfully prepares it.

When I try to start up L4D, everything is normal up until the point when L4D would actually execute. It never does, and gives me the useless "Left4Dead has crashed, please send Windows an error report blah blah blah send or don't send" message. Doesn't give us a lot to go by, and the problem is especially vexing because it started spontaneously, after about a week of playing with no problems, and without me changing anything on my system whatsoever since install.
So to answer your question, it doesn't cause a restart or a blue screen or anything. The program simply won't execute normally.
EDIT: Just found out that the problem isn't just with L4D, but all games running the Episode 2 engine.

i got similar prob but i get message of " this game is unavalable please try another time i have uninstaled etc its retail version i dont think this is prob as it was working
someone did mention deleting registry can anyone help me do this pls i have vista 32b home edition
nvidia 8800 gts with latest drivers
intel quad
coasiar memory 1333fbs
pls help if you can
