EG, build a worker, then your capital builds nothing but settlers until you're humping your rival's borders. New cities build a worker, then highest defensive troop or two, then buildings. S'usually how I do things.

i'd suggest against always expanded, you may overexpand and thus dropping research to net to nothing and falling behind tech wise.
The simple way is if your researching at higher then 80% then you should look to expand. Obviously there are times when it is more important to expand, different levels etc.
2nd it helps to try and make sure you are at least mid level on the power chart (if i recall correctly) if you are towards the bottom, likely your going to find yourself being attacked by the AI since they know you are weak. Obviously the empires around you matter as an aggressive leader is more willing to attack then others.
As for how long, for me its usually a lonnnggggg time as i tend to focus on winning through culture first, war if i get the chance but i tend to defend more. I don't use time (There no end date in real life (yet)), or Space race (Stupid win condition imo). Got to win either by conquest, domination, culture or Diplomatic

I did do something magical, I killed off a computer with immortals and catapults. Of course that left me economically fucked and at the mercy of the 10 or so other computers. So i essentially lost the game right afterwards. Still a personal best.

My fastest win (without advanced start) was by domination around 1650 or so. I racked up the points for that one!
Usually I finish in the 1990's or early 2000's via domination. I have only done conquest once.
EDIT: I remembered that one time I got a diplomatic victory (via the Apostolic Palace) around 500AD which was ass kicking.
