How old is the card? What brand is it? Is the card under warranty?

I've twice had this issue with a video card fan.

In one case using compressed air to blow out accumulated dust and crap was sufficient.

In the other, I was able to take the fan out and lubricate the bearing. It wasn't dusty at all, but it was old and the lubricant had (presumably) dried out. It wasn't sticky, but the fan motor didn't make enough torque to spin it up. I used a very light oil (approx. sewing machine oil) and that completely fixed the problem. The type of lubricant you want depends heavily on the type of bearing. Some bearings won't take lubrication, and in those cases you may need a new fan.

I'm not saying that this is what happened, but don't ever run your video card fan at high speeds, unless you're gaming. You're just killing it quicker if you do.
