Save it to a jump drive. Start the infected computer up in windows safe mode and install it and run it...
    Works like a charm...

It's effective MOST of the time, but my in-laws' PC got infected with a nasty virus that included a rootkit and it won't allow any connections to ANY website/ftp site where the definitions are stored. All web browsers refuse to start, etc. Only option is to reformat. I've tried everything else. Really evil stuff out there and my father-in-law seems to find it all.

There's another option to reformatting: use the disc image you very wisely created a few days ago to revert back to a fully functional PC.

Do it now, all of you. Image your finely tuned PC and you won't ever have to reformat and re-install again. I use Macrium Reflect Free, but there are other free and paid alternatives out there. Schedule it to run regularly, and quit worrying.
