    Did it this morning:

    On No Mercy, before the final chapter, I was in the red and limping and my teammates were already in the Safe Room. I could hear a witch nearby but was not sure where it was. As I was rounding the corner to the Safe Room door, a teammate closes it and BAM! the witch was behind it - I startle it (shit my pants) and I get creamed.

    On the bright side, somebody got the Achievement for coming out of the Safe Room and saving my sorry ass
