One of those should be in the 'server' config file found in the steam > steamapps > common > left 4 dead > left4dead > cfg folder named server yes... along with the sv_gametypes.

Here is the relevant section from mine...

//Gametypes sets the gametypes you want your server to run
//----> Versus Only: sv_gametypes "versus"
//----> Survival Only: sv_gametypes "survival"
//----> Coop Only: sv_gametypes "coop"
//----> Both: sv_gametypes "versus,coop"
//----> All: sv_gametypes "versus,coop,survival"
sv_gametypes versus,coop,survival

//mp_gamemode sets the initial state your server will be in
//----> Versus Only: mp_gamemode "versus"
//----> Survival Only: mp_gamemode "survival"
//----> Coop Only: mp_gamemode "coop"
mp_gamemode coop

sv_gametypes versus,coop,survival to allow versus,coop,survival play
mp_gamemode coop to set the initial state your server will be in to coop.
Steam will change the game state automatically to correspond to the game assigned to your server.

So you will have to have the sv_gametypes versus,coop,survival and the mp_gamemode coop in there in some version.

You may have to edit the mapcycle file found in the steam > steamapps > common > left 4 dead > left4dead folder and add/delete what you like.

The maps to add are found in the maplist file in the same folder.
