They're related, but it can be fair while being unbalanced - see my example above for something that's absolutely fair and also absolutely unbalanced.
At the level we tend to play at TTP, VS works pretty good. I think it'd be more fun (and a fuckload less frustrating) if things like defibrillators were removed (or simply changed so that once someone is dead, they no longer count for points even if they're revived) and items in general were reduced in density, but it works alright.
At the competitive level, though, vanilla VS is completely, utterly broken in favor of the Survivors... and even with all of the changes comp mods add, at the highest levels of play, it's still a little in the Survivors' favor.
