    I am requesting a reserve slot for 2 months at a cost of $5.00 per month to the following servers:

    I have held a reserve slot in the past and understand the addtional responsibilties that comes with being a reservist. I futher understand that no refunds are given. I also agree to follow all TPG rules & regulations.
