Triple the common populations, never-ending rushes and more specials at one time. Every time a special attacks you, it would spawn more hordes.

Quick 2 Aid- Similar to regular realism. The difference is that human survivors only have 100 health when hanging off a ledge, or when incapped. Better find them and revive them quick before they die!
Rush!- Campaign with endless hordes of zombies chasing you to the saferoom. You'll have to fight through mobs that never stop coming, along with normal specials and crescendo challenges.
Healthpackalypse Co-op- Similar to Healthpackalypse mutation in versus, you have to complete the entire campaign with no health items of any kind, including defibs. Survivors do not respawn at any point in the chapter, even when a new level is loaded.
Accuracy Pays- All guns have their accuracy reduced by 50 % in this campaign. This means that Magnums, Rifles, etc. have double the crosshair size when moving, similar to the M60 and the old Hunting Rifle.
Hardened Realism- A unique game of realism versus. In this RV game, music and subtitles are disabled for the survivors at all times. In addition, there's no ammo piles in the level, just like Iron Man, only more weapon spawns. Finally, no temporary health appears, meaning your only source of health is the four medkits you get at the start, and the four at the ending saferooom... if you can make it that far.

Change is Good- Pretty much the opposite of the L4D1 mutation. You only have L4D2 infected and the new weapons. No smokers, pump shotguns, etc. Regular commons and regular pistols do appear.
Nightmare Versus- A version of the "Nightmare" mutation, with increased fog, and a black sky in every campaign, for versus mode.
Bleedout Scavenge- Same as normal scavenge but with only temporary health. Intended to provide quicker scavenge rounds.
I Don't Play God- A mutation available for versus or campaign. In it, there is an extra fifth/ninth player who is selected in the lobby to act as the "human director" for the game. They are able to fly around in spectate mode, and place items such as first aid, weapons, ammo, and throwables wherever they want on the map. Each button is programmed to spawn something. They can spawn tanks and witches, decide when hordes come, and decide which cars are alarmed.
Just Like Counter Strike- Campaign, but your only weapons are the normally German exclusive ones ported from Counter Strike. These can be seen here-
International Weapons - The Left 4 Dead Wiki - Left 4 Dead, Left 4 Dead 2, Survivors, Infected, walkthroughs, news, and more . You start out with the SMG and Scout, then later you unlock the other two guns as tier 2s. Pistols remain in the game.

Ultra Realism VS: Similar to Special Delivery but with a few rather nasty tweaks. the amount of CI is significantly larger than standard VS along, multiple witches that are spread out evenly across the map, and up to 2 tank spawns per map. AI SI will continually spawn and attack like in Special Delivery but only up to 3 will spawn. To counter this, survivors get...nothing.

Silence is Bliss - Every gunshot, thrown pipe bomb/molotov, and yell a survivor makes attracts a horde of Common Infected. Survivors will always speak in hushed tones like when there is a Witch around.
Tear Gas - Survivors must travel to the safehouse under the effect of smoker gas. Very few Infected, but survivor health decreases by 3 when they are at green health, 5 when they are at yellow, and 7 when they are at red. No health packs, and and pain pills have reduced effect.
