I've been playing for years and have yet to witness it. Does the commander get credit for a kill when that happens?

That i don't know but i seem to have a nack for shit falling on me. I dont even want to think about how many times a helo or jet carcass landed on me and killed me. The body deal has only been 1 time. Unless you want to count people jumping off buildings and such then landing on me and killing me.

Well, I know that jumping out of a blackhawk and falling into a squadmate with a parachuting causes him to either simply die, or the parachute gets tangled up and he falls 300m to his death. Both ways, that's a teamkill

Parachutes will kill anything that touches them in-flight; not the crate/jeep but the canopy itself. I've seen aircarft blow instantly upon touching the canopy. Hit the cargo and it swings wildly. Hit the canopy - BOOM.

Same for troops.

NEVER follow a teammate over the side - you will die as soon as you touch his canopy and earn him a TK. Either delay your jump or jump slightly to one side.
