md5tool, I believe, is like md5sum. It's provided as a part of PunkBuster to check if files have been tampered with or are corrupt.
md5sum is a utility that, when ran against a file, produces a 128-bit Message Digest 5 hash. A list of known good hashes are added to a text file and normally saved on the server. These hashes are like a fingerprint and it's used to check the validity of a file. If the hash doesn't match to what is listed in the text file, the file is not the same; it's either corrupt or a hacked copy of the file.
A good example of how it's used is when downloading an ISO image of, say, a Linux distribution. If you go to this URL, you'll see a list of ISO images for Debian GNU/Linux. Look at the MD5SUMS file which stores the hashes that were ran against the ISO image files. This is the list of known good hashes. After you download one of the ISO images, you run md5sum <filename> against the file and it'll calculate the MD5 hash. You compare that to what's in the MD5SUMS file. If they match, the file you downloaded is fully in-tact and an exact copy of what is stored on the server. If the hash doesn't match, the file you've downloaded is not an exact match and is most likely a corrupt download.
