Usually, one person is making the calls. Switching it up every round, while possible, usually just isn't done because it would most likely result in multiple calls, and thus confusion. However, if someone else wants to take over the reigns and start to make calls, then it is up to them to contact the current leader(when you're both dead is preferable, and is more efficient and less distracting than when alive), and ask them if it is alright to take over the shotcalling. More often than not, that leader won't mind stepping down so another leader can step up. In fact, changing leaders midway through the map when you're on the objective side is good strategy, as the new leader usually calls some different strategies than the first, and it will catch the defensive team off-guard. That's really something I think should be employed more often.

I like to "pass the reigns" on to someone else halfway through just because it is a little mentally exhausting to continue to try and outsmart the other team for the duration of a map (especially if it gets extended). Me and Scrum & Me and Jeff play switch-a-roo quite often.
