Financial questionnaire help

Group,

I need your help filling out a questionnaire I created on survey monkey. Some of you may already know, but I work in a community bank and underwrite loan requests for a living. I have an idea I think will make life easier for everyone but in order to confirm my thoughts I created a survey and I'm really hoping to get some honest feedback on it. This is my first survey, I'm not a statistician or an English expert. I think I have what I need here to reach a conclusion so if you could help me out an share it with your friends and family you'd really be doing me a big favor.

This thing takes maybe 10 minutes if you're drinking coffee casually, or have interruptions. As an incentive for helping out I'm going to be selecting a few random winners to receive a $5 gift card for their efforts. Ha, I underwrite loans I don't approve nor deny them. I'm the prick in the back asking all these absurd questions. Well, I try not to be a prick.

Oh sorry, let me rephrase, If you were to be unable to pay how long would I have to allocate on an hourly basis to hold you underwater?

Consumer protection on bad loans is absurd these days. That may be the only way to get your money back on bad investments.

For the degree of schooling option make sure to have an option for 'higher degree than listed'

for #13 have an N/A option. (If you own a small business, how difficult is it for you to receive financing for major purchases whether it's for the business or you personally?)

Appreciate the feedback guys. Yeah tbh the education thing is just a demographic deal. To be entirely honest with you guys there's not much of a correlation between bad investments and education. I can't tell you how many MDs, MBAs, and CPAs I've seen make bad loans. I really just wanted to get a feel with what I'm working with.
