Hanging for 10-20 seconds ever few mins
Well the title says it all really but weirdest thing is, I hadn't done a thing to my system since last night when I was running it fine with no problems apart from the company I was with lol.
I do recall though I had this same problem when I first purchased the game and first ran it. Alas for the life of me I can't remember what I did to fix it. I checked for new Gfx card drivers but I have the latest ones, I set Nvidia control panel to factory settings as they were custem set and I reverted L4D settings back to recommended instead of max.
The only thing did happen to my system is a scheduled AVG scan I have at 10am ever morning and then I had auto shutdown when it was finished. I started it back up the next day(today) and now I have this problem.

Well, the evidence seems to point to the AVG, though I am not sure what could have happened. Do you get an alert when AVG detects a "virus"? Is there anything in quarantine the belongs to L4D?
You might try verifying the integrity of the game files via Steam, but I am not confident that it will help, but it certainly can't hurt.
And, you have made sure that you don't have any other programs or utilities that might be kicking off, or running while you are trying to play?
Try launching the game, and when it starts to act up, hit your Windows key to minimize the game, then go to your task manager and see if there is something that is stealing CPU cycles.*********************************

I found a solution, I fiddled around with the setting in the L4D video menu and i fixed it although I have pin pointed the exact cause. The texture option are fine I reverted them back to max but it seems to be 1 of the settings above those IE anti analyzing/multicore etc i reverted all them back to none or disable despite the fact that they were at default settings.
I shall figure out a which option it was and let you all know for the benefit of future indevours for other ppl. I do have a sneaky suspiction that it might be the disabled multicore since I have another game tha euns better with that off but I will check anyone way.
