This is my formal request to be added to the reserve list for the TTP CS server.

I understand and agree to all terms and conditions. I was interested in setting up a 1 month Reserved slot on the CS:S Main server and a 1 month Reserved slot on the Top 5 server. I understand that this will be a one time fee of 10 dollars, and it is non refundable.
