I have read the server rules, and agree to abide by them. I also understand that for a slot a $5.00/month donation is required, and no refunds can be given. I would like to request a 3 month slot for a minimum of $15.00 on the CS:S servers.

no problem. Remember, the slots autorenew themselves, so if you only want to do it for 3 months, simply cancel it in paypal before your 4th payment.
