Gonna focus on narrow-minded medics here. Something I've noticed but, not all the time, but alot I've been seeing medics focusing their "heal-beam" onto a single player and ignoring the rest's medic call.
As a medic, it's your choice to give life or prevent it. It's about making sacrifices, but you have to realize that there are seconds between battles where your primary target (the person you focus on healing) is healthed up and you can spare a couple of seconds to heal other teammates. It's not necessary to fully heal members of your team. If you are the the midst of a battle and you see a teammate in red life why not heal them half of their life and proceed on carrying out with your primary heal target (which should have 150% of their normal life). If you get lucky then you heal you secondary target enough to survive the battle, have you primary target fully healed, then if you have time go to you lesser casualties and fully heal them. By doing so, you have kept your ubercharge going AND you have members of your team able to potentially advance or kill flankers trying to wipe out the medics.

I am a relative rookie at TF2, but medic is one of my most played classes because it seems like my team rarely has enough, and I am willing to switch. I agree with this statement, but will add that if I am the only medic, and I have found someone that not only works for the team trying to push the objective, but also makes sure I can get back to get meds when needed, I am not going to leave that person to trek all the way across the map to heal someone that has no clue.
I know this is not what you are speaking of, but last night people kept asking where is our medic, notice singular on a full server, and my reply was we need more than one. If you keep getting killed without a medic in sight that is probably a sign you need another one.

Spread the love around as much as you can, don't wait for someone to call medic, just heal anyone. Of course this only applies when the fighting isn't heavy, once you head into the fray, it's good to stick with someone.

idealy there should be enough medics so you have 1 or 2 focusing on healing 1 or 2 people in the middle of battles.. while a 3rd medic rotates around the back healing anyone who needs heals.... now this isnt always the right way.. nor does it always happen.
However a medic needs to be spreading the love around... if a guy is full health and isnt fighting atm.. check around you for others who need help.

Working with medic is a two way street.
Like all things in this game it takes teamwork.
Primary target needs to know when to hold up to give medic time to heal others.
Others need to come to the medic, not expect him to risk his life to come to them.
Medic should vocalize HIS location to the team to facilitate this behavior.
Exp - When I am medic I will often make the call "Medic is right side tunnel" or "Medic is second floor house".
People are trained to run where they know health is, so let them know where you are!

If you feel like suicide tactics are necessary, tell the medic who's kind enough to heal you so that he doesn't follow you in and get killed too.

I have to say that I think if anyone needs a headset in the game it's a medic. They can announce a spy behind a heavy, tell the guy they're healing that they're about to uber, or best of all, tell the guy that they're healing to hold on a sec while you juice up the rest of the team. Don't just peel off of him and let him die. Communicate communicate communicate.
